Athletic Forms and Info

All students who plan to participate in athletics at The Donoho School must have a physical evaluation performed and signed by a doctor.  You must have physical before you can participate in practice. Physicals are only valid for one year from the date when it was taken.

Dragonfly MAX -

Dragonfly MAX is the new AHSAA software designed to make the form filling out process easier.  Please visit the website and sign your child up.  This must be done before any athletic activity on campus.  All of the forms are now digital, and can be signed for online using the Dragonfly Max software.

NFHS Sportsmanship Initiative:
The Alabama High School Athletic Association (AHSAA) Central Board of Control approved the NFHS Sportsmanship Course and added the program to student eligibility requirements. Students in grades 7 through 12 MUST complete the on-line interactive program and meet all other established requirements in order to be eligible for high school interscholastic athletics, including cheerleading. Students will need to create a NFHS account, complete the course, and print the certificate of completion to return to the school to file in their eligbility file. 

Orthopaedics and Sports Medicine services are provided to the school by Clinton M. Ray, M.D. Orthopaedics and Sports Medicine, LLC.
