Athletic Forms and Info

All students who plan to participate in athletics at The Donoho School must have a physical evaluation with a doctor on file in the school's office prior to participation in any practices or competitions. Physicals are only valid for one year from the date when it was taken.

A parent must fill out and sign all pages of the four forms listed below, except the second page of the AHSAA Pre-participation Physical Evaluation which the doctor will fill out.

NFHS Sportsmanship Initiative:
The Alabama High School Athletic Association (AHSAA) Central Board of Control approved the NFHS Sportsmanship Course and added the program to student eligibility requirements. Students in grades 7 through 12 MUST complete the on-line interactive program and meet all other established requirements in order to be eligible for high school interscholastic athletics, including cheerleading. Students will need to create a NFHS account, complete the course, and print the certificate of completion to return to the school to file in their eligbility file. 

Orthopaedics and Sports Medicine services are provided to the school by Clinton M. Ray, M.D. Orthopaedics and Sports Medicine, LLC.
