Athletic Forms and InfoFORMS FOR PARTICIPATION:
All students who plan to participate in athletics at The Donoho School must have a physical evaluation with a doctor on file in the school's office prior to participation in any practices or competitions. Physicals are only valid for one year from the date when it was taken.
A parent must fill out and sign all pages of the four forms listed below, except the second page of the AHSAA Pre-participation Physical Evaluation which the doctor will fill out.

Orthopaedics and Sports Medicine services are provided to the school by Clinton M. Ray, M.D. Orthopaedics and Sports Medicine, LLC.

The Alabama High School Athletic Association (AHSAA) Central Board of Control approved the STAR Sportsmanship Initiative and added the program to student eligibility requirements, beginning with the 2008-2009 school year. Students in grades 7 through 12 MUST complete the on-line interactive program and meet all other established requirements in order to be eligible for high school interscholastic athletics, including cheerleading. The school code is STAR02068.
